Media Release & Public Use Authorization Form

Allows you to decide whether portions of your recording, photographs, audio clips, video clips, testimonials, or transcript excerpts may be used publicly. Participation is entirely optional and will not affect your services.

Minor Participation & Parent/Guardian Consent Form

Required only when the participant is under 18 years of age. A parent or legal guardian must complete and sign this form before the interview.
